Meet Our Social Prescribing Team

Social Prescribing Team

At Audley Health Centre, our Social Prescribing Team consists of Margaret, James, Katie and Carol.

Our Social Prescribers are Link Workers who work with patients to improve their overall health and wellbeing by connecting them with local services, community groups and organisations that can provide practical, emotional and social support.

You may benefit from seeing a Social Prescriber if you need help with:

Feeling lonely or socially isolated.
Finding local social groups, community activities or support groups.
Mental health and emotional wellbeing support.
Debt, benefits or housing advice.
Practical household support.
Accessing services from Social Care, community pharmacies, Allied Health Professionals, the Fire Service, housing associations, Jobcentre services, employment support or volunteering opportunities.

Our Social Prescribers take the time to understand what matters most to you and can help you find the right support to improve your health, wellbeing and quality of life.

How to request an appointment

If you feel you would benefit from speaking with a member of our Social Prescribing Team, you can:

Submit an online consultation request.
Complete the online referral form below.
Contact your GP surgery reception and ask for an appointment with a Social Prescriber.

When contacting the practice, please let us know why you would like to speak with a Social Prescriber and provide the best telephone number to reach you on. One of our Social Prescribers will then arrange an appointment with you.

We are here to help you access the right support at the right time, so you can make positive changes that improve your health and wellbeing.
