Appointments

Click on the submit button below to send an appointment request to the practice. Medical requests can be submitted Mon-Fri 07.00-18.00.

You can still call the practice after 18.00 if your medical issue is URGENT.

We run all our surgeries by appointment only and a face to face or telephone consultation will be offered.

All GP and Advanced Nurse Practitioner appointments will be triaged by a clinician. The reception team will no longer make these appointments for you.

For all patients that don’t have digital access, the reception team can complete your request and submit it for you.

Please ensure that the team have your up-to-date contact information. As this is a new service, we anticipate that they may be some teething problems initially, but please be assured that that the team will work hard to rectify any issues as soon as possible and that our new system is monitored and tweaked to ensure the best possible service to our patients.

Patients can submit their online between 7am-6pm Monday to Friday. Requests will only be reviewed by a clinician within these times. Outside these hours, requests will be triaged the following working day.

Practice Nurse and Health Care Support Worker appointments will not need to be triaged and you can book them with reception team as normal.

  • You can book an HCSW or Practice Nurse appointment by phoning the surgery on 01782 276999 or visiting the surgery in person.
  • You can request an appointment online through the NHS App or Patient Access (Contact GP about an health problem) Please speak to one of our Receptionists if you require any assistance
  • You can complete an online form for medical/admin queries
Once you submit a form online, the surgery will forward your query on to our triage team, who will send a prompt response.
We try to make sure you see the doctor of your choice. If they are not available, we may ask you to see another doctor.
 

Routine Appointments

To request AN NON URGENT APPOINTMENT WITHIN THE NEXT 1-2 WEEKS.

  • The Practice Team will review your request. You will be contacted to either make an appointment, or sign post you to a more suitable service.

  • You may also be offered an appointment with our enhanced hours team. This will be an evening or weekend appointment.

  • Please do not submit urgent medical requests through the admin query option

Same Day / Urgent Appointments

To request AN URGENT APPOINTMENT FOR THE SAME DAY.

  • Select the medical request section and submit your request.
  • The Practice Team will review your request. You will be contacted to either make an appointment, or sign post you to a more suitable service.

  • You may also be offered an appointment with our enhanced hours team. This will be an evening or weekend appointment.

  • Please do not submit urgent medical requests through the admin query option

Your Appointment

We will offer you the most appropriate type of appointment dependent on your request. This may be:

  • By phone

  • Face to face at the surgery

  • On a video call

  • By text or email

While patients may prefer a certain type of consultation, this may not always be suitable.

Please give us as much information as possible when submitting your request. This allows us to triage your request, and book the correct appointment.

Home Visits

If you are too ill to come to the surgery and you require a visit at home , please select the medical request section and submit your request before 10am. Home visits take up more of a doctor’s time, so if you are mobile please come into the surgery to be seen.
 
You may be offered a home visit by the Primary Care Network Paramedic who works in conjunction with the practice who will liaise with the doctor on call about your care.
 

Extended Access – Late Evening & Weekend Appointments

As part of Newcastle North Primary Care Network, we deliver extended hours on a Saturday. These hours are delivered on a rota basis between our five practice, Audley Health Centre, Heathcote Street Surgery, Dr Harbidge & Partners ,Mount Road and Talke Clinic. This will enable our patients that work to access appointments on a Saturday rather than having to take time off work. If you are attending one of the other practices, you will be asked for consent for the clinicians to access your medical records via the EMIS computer system. If you do not give consent, you will not be able to be seen.
This service is available to all registered patients.

Chaperones

We understand that you may wish for a chaperone to be present during your consultation.
 
Please ask one of the receptionists prior to your appointment. Our clinicians also have the right, in some circumstances to insist on a chaperone to be present.
 

Changing an Appointment or Cancelling

If you cannot attend an appointment for any reason, please inform us as soon as possible in order for us to give the slot to someone else.

  • Through the NHS App / Patient Access
  • Via the link on your appointment reminder text message

  • Complete our online form (48 hours before your appointment)

Non Urgent Admin Request

  • Select the admin query section and submit your request.

You can request the following:

  • Letters from the GP
  • A continuation/extension of your current fit or sicknote
  • An update on your referral
  • A repeat prescription / medication information
  • Get information for test results
  • Have a non urgent medical request
  • Have a query about something else

 

Frequently asked questions

Why have you changed the appointment system?

This online system is a simple , flexible and fast way to contact the practice with medical or admin queries. Patients are already using the service for non-urgent admin enquires. We are then able to triage these requests to be dealt with by the most appropriate team at Audley Health Centre. Online consulting allows you the flexibility and not in a queue at 8am on the phones, it is very simple to fill out but if you do have difficulty contact the practice by phone and we can talk you through this or complete if necessary.

We understand this is a big change but a necessary one to allow us to provide the best and safest healthcare we possibly can.

We understand the frustration you feel when trying to book an appointment to speak to an clinician and the time taken to get through to the practice. We have listened to your feedback and have decided to move in a direction which is Total Triage.

Total Triage has been rolled out in GP practices across the country with positive feedback from patients.

This replaces our current system of telephoning the practice at our busiest time from 8am when our phone lines open to get an appointment.

 How does  the online booking system work at Audley Health Centre?.

Our online consultation system opens at 07.00 am.

You can contact us using the admin request option anytime during any working day. This is for non-urgent requests only.

What if I do not have access to the internet or I am not well versed with how to get online?

If your personal circumstances mean that it is impossible for you to do this our care navigators team will be happy to submit your request on your behalf.

Patients can contact the surgery by phone or walk in, and a member of the team will support with their request.

For those that are vulnerable or housebound, and unable to get to the surgery, we can offer home visits. Your GP will visit you at home if there is a sufficient evidence that your medical condition warrants it and you cannot make it to the surgery.
