Sick notes can only be issued when they are due, although they can be back dated they cannot be future dated. Please do not request emergency appointments for a Sick/Fit Note.
Off work sick 7 days or less
If you are off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

More than 7 days off sick
If you are off work sick for more than 7 days, your employer will usually ask for a fit note or statement of fitness for Work from a healthcare professional.
If you require a current sick note to be extended, please note this can not be issued until the date of your current one which you can request by contacting the surgery or complete an online consultation query form.
Sick notes covering period of stay in hospital and expected recovery time
It is the responsibility of the hospital staff to issue sick notes for a period of stay in hospital and the ‘expected’ recovery time afterwards. This certificate should be obtained before you are discharged from hospital.
