Social Prescribers are link workers that have access to a wide range of local agencies, best suited to help with your overall health & wellbeing.
These are voluntary and community sector services that could help support a wide range of needs including: loneliness, social groups, mental health, support groups, practical household support, debt, benefits & housing.
Patients can also be helped to access services from social care, pharmacies, allied health professionals, fire service, housing associations, job centres, employment and/or volunteering support.
